1- New Registration Fees:

Effective August 1, 2009 the above fees will be subject to a late payment fee of 40.00$

Fundraising tickets (60.00$) are included in the registration fee, maximum 2 booklets per family.

3- Payment

Registration must be paid in full before a player participates in on-ice activities. Cheques may be post-dated for October 31, 2009. Registration will not be accepted unless payment is attached. NSF cheques will be subject to a 25.00$ charge.

VISA and MASTER CARD accepted.

4- Registrar

Our registrar is Yves Poirier. Registration forms will only be accepted by him. He must sign all forms. Please complete all sections of the form. Registration forms and payment can be mailed to ADMHA, P.O. Box 1054, Alexandria, On, K0C 1A0.

Forms are available at the arena or from our website at admha.ca

Proof of residency may be required: Tax bill, Hydro bill or drivers license.

5- Waiting List

All players registering after August 1, 2009 will be placed on a waiting list.

If your child is trying out for a team outside our association, he/she must register with the ADMHA first. We will accept a post dated cheque for September 30, 2009. Please notify the registrar Yves Poirier @613-525-1307 as soon as you know where your child will play.

7- Out of Township Fee

Players not residing within the association's boundaries will be subject to a 75.00$ charge.

8- Fitness Tax Credit

Receipts will be issued only when your registration is paid in full.
